What skill are you using? % life steal only works on physical weapon damage. If you're using a skill that converts all the damage to some other type, it won't work. Elemental melee builds (including Unholy Paladin skills) need to use Life on Attack instead, which works with any melee attack.

9/26/17 - Monthly, quarterly, and annual median and average sales prices, sales price distributions and price indexes have been corrected for October 2014 through January 2017 to include additional response data that had been omitted from previously tabulated estimates. Monthly median and average prices were revised downward about 1.2% on average. U.S. total quarterly and annual prices had somewhat larger downward revisions of up to 4.2%. All corrected estimates are available on our website.
Seasonally adjusted estimates for January 2011 through March 2013 have been revised. Estimates of median months for sale in Table 3 for January 2010 through March 2013 have also been revised. New software has been implemented to estimate medians. Research indicated that the previous software overstated the recent estimates of median months for sale by up to 3.1 months.
Cascade (Perflow Medical, Netanya, Israel) is a novel, retrievable, neck-bridging device designed to assist in coil embolisation of cerebral aneurysms. Cascade, like other neck-bridging devices, may serve as an alternative to balloon-assisted techniques, with the added advantage of providing patency of the parent vessel during coil delivery, thus reducing risk of complications associated with ischaemia. Sirakov et al report an early 2019 clinical experience using Cascade with coil embolisation for treatment for 12 ruptured wide-neck aneurysms. Complete occlusion was observed in 75% (9/12) patients, and no patients received antiplatelet therapy. No technical or clinical complications were observed during treatment; however, the study did not report follow-up data. The Cascade is similar to Comaneci (see section dedicated to Comaneci); however, Cascade is compatible with a smaller 0.017 microcatheter better suited for more tortuous vessels. Cascade features denser net and deflected shape during expansion unlike Comaneci. Sirakov et al12 report less coil entanglement and better compliance with parent vessel during expansion with Cascade compared with Comaneci. The authors acknowledge several limitations of their study, such as the small sample size and lack of long-term follow-up. The higher rate of complications for patients undergoing endovascular coiling for wide-neck aneurysms remains an important issue, and early data for the use of the novel non-occlusive Cascade device appear promising and warrant further safety and efficacy studies.
A three-round design has been selected to maximise the chance of delivering consensus, while minimising the attrition bias that the survey is exposed to if too many rounds are undertaken. In the general medical literature and in the developments of core outcome sets, the median and upper quartile number of rounds completed is three rounds,44 45 with only 6% of Delphi studies used to agree importance of outcomes incorporating more than three rounds.46
At second and third rounds of the Delphi Survey, panellists will be provided with median scores for the whole group and a graphical representation of the score and interquartile ranges. Individual stakeholder group sub-group analysis will not be performed here but will be performed and provided to participants in the consensus meeting.
